Targeted physiotherapy for thumb-sided wrist pain and tendon irritation

de Quervain’s Syndrome Gold Coast

At Evergreen Hand Therapy, we provide evidence-based de Quervain’s Syndrome (also known as de Quervain’s Tenosynovitis) treatment focused on accurate diagnosis, targeted splinting, and progressive rehabilitation to restore comfortable movement.

Led by Dr Lauren Miller, Accredited Hand Therapist with a research PhD on finger fracture management and over 20 years of clinical expertise, the clinic offers individualised assessment and management for people living across Varsity Lakes, Robina, Burleigh, and the wider Gold Coast. Treatment focuses on reducing pain, restoring thumb motion and strength, and maintaining confident hand function in everyday activity.

de Quervain’s syndrome is an irritation of two tendons that move your thumb as they pass through a small tunnel at the wrist. When these tendons become overloaded or inflamed, or the roof of the tunnel thickens, the tunnel becomes tight. This creates pain at the base of the thumb, swelling along the wrist, pain when gripping, lifting, or wringing, and discomfort when lifting a baby or using your phone.

It is often referred to as “mummy’s wrist” or “mummy’s thumb” because it commonly develops in new parents from repetitive lifting and supporting a baby. It is also common in tradespeople, office workers, and anyone performing repetitive thumb and wrist movements.

Diagnoses: A clinical assessment is usually sufficient. Imaging is not always necessary unless symptoms are unusual or not improving. We assess pain location, tendon irritation, swelling, grip and thumb strength, and how painful certain combined movements are (including Finkelstein’s test).

Treatment Options

Splint or Brace Support: At Evergreen Hand Therapy, we fabricate custom splints when needed, ensuring the right balance between protection and movement. Splints must be prescribed correctly, as over restricting the thumb for too long can lead to stiffness and weakness. A well fitted splint or brace can rest the irritated tendons, reduce friction in the tunnel, settle inflammation, and allow healing to begin.

Exercises: Rest alone is rarely enough. The tendons need structured, progressive load to remodel and recover properly. Once pain begins to settle, targeted exercises are introduced. These focus on gentle tendon gliding, gradual loading, and strength restoration.

Taping: Taping is typically used alongside rehabilitation rather than as a standalone treatment. Supportive taping can reduce strain during activity, provide short term pain relief, and assist transition out of a splint.

Injection: A corticosteroid injection may be recommended by your Doctor in some cases, particularly when pain is severe, symptoms are persisting, or conservative care has not settled symptoms. Wearing a splint or brace support, in combination with an injection, can lead to better function and less pain, compared to just having an injection alone.

Surgery: Surgery is usually considered if symptoms persist despite comprehensive therapy or if the pain significantly limits function. Surgery involves releasing the tight tendon tunnel. Recovery still requires guided hand therapy rehabilitation.

How Evergreen Hand Therapy Can Help

Each treatment plan is designed specifically for your presentation and lifestyle demands. Our aim is to restore comfortable movement, protect long-term tendon health, and support you in returning to daily activities with confidence. Care may include:

  • Comprehensive clinical assessment
  • Custom splint or brace fabrication
  • Manual therapy and structured exercises
  • Taping for activity support
  • Education and load modification advice
  • Graduated return to work, sport, or parenting tasks

 Frequently Asked Questions

Sometimes mild cases improve with activity modification. However, many people find symptoms return once they resume normal tasks, pain persists for months without structured care, and grip strength declines. With proper therapy, most people recover well and return to full function without ongoing limitations.

The earlier the condition is addressed, the easier it is to settle. You should seek assessment if:

  • Pain lasts more than 2 to 3 weeks
  • Lifting or gripping is painful
  • You are avoiding daily tasks
  • Night pain is developing
